When the world is raining down on you, stop under the umbrella of AASC's Care
Coordination program. Care Coordination enables older adults to remain at home
as long as possible. This program links you and your caregiver to a broad array
of services and benefits (inside the agency and out), Care Coordinators and the
Information and Assistance Specialist, acting as advocates, can connect you to
services provided by AASC, including personal care with bathing, dressing,
feeding, and light housework. Other services they can coordinate include
medication management, case management,
telephone reassurance,
disease prevention/health,
VICAP,
SMILE program,
emergency services,
emergency
home repair,
fuel and energy assistance, and
nutrition. They can also help you obtain non-AASC services, including
assistance with getting glasses, hearing aids and dentures. Care Coordinators
assist with Medicaid questions, make referrals to centers for independent
living, and in general, try to find the answers to your questions. We also have
a program that recognizes the birthdays of centenarians.
You, your family member, a friend, a home health agency, or even a hospital
employee, will contact AASC about an apparent need. If the need is of a short
term nature, the Information and Assistance program will help. If you have a
greater need for assistance, a Care Coordinator will visit your home to see what
types of assistance you need in activities of daily living. If the Care
Coordinator finds that you do need assistance with at least two activities, and
have unmet needs, you may receive Care Coordination services. In coordinating
services and/or benefits for you, Care Coordinators will contact you at least
once a month by phone or by home visit.
Information and Assistance programs include Farm Market Fresh, Get Alarmed
Virginia!, fall prevention, New Eyes for the Needy, affordable dentures, energy
assistance, and help with Medicare Part D.
