PACE (Program of All-Inclusive Care for the Elderly) is a Program of the Commonwealth of Virginia.

The AllCARE for Seniors PACE program offers a long-term care solution for your loved one. By coordinating, providing, and supervising all the home and health care services your aging family member needs, we help the one you love stay safe and comfortable at home. Our mission is to enhance the quality of life of frail, older adults and support their independence. AllCARE enables your family member to live at home as long as it is medically and socially possible. By doing so, we help preserve the dignity of the older adult and maintain the family circle.

Who is eligible for AllCARE for Seniors PACE? Individuals:

¨ who are age 55 years or older, and

¨ who live in Buchanan, Dickenson, Russell, or Tazewell County, and

¨ who meet nursing facility level of care but are able to live safely in the community.

AllCARE for Seniors provides services to participants at home, at the PACE center, and in the hospital. Our interdisciplinary team of health care professionals includes a primary care doctor, registered nurse, therapists, Master’s-level social worker, pharmacist, dietitian, home care coordinator, transportation coordinator, personal care attendants, and PACE center manager. Medicaid, Medicare, and private-pay options are available to AllCARE participants.

AllCARE for Seniors services include the following:

¨ Doctor Visits, including care by a network of community primary care physicians and specialists

¨ Hospital Stay and Emergency Services

¨ Prescriptions

¨ Physical and Occupational Therapies

¨ In-Home Support and Personal Care

¨ Adult Day Care

¨ Social Activities and Recreation

¨ Meals

¨ Transportation

When your loved one enrolls in AllCARE for Seniors, our interdisciplinary team provides and coordinates ALL health care services. The health care team must approve all services, except emergency care. AllCARE for Seniors will not pay for unauthorized, non-emergency services. Participants may disenroll from AllCARE at any time and continue their usual Medicare and Medicaid benefits.
